REQUIREMENTS FOR DUAL CITIZENSHIP PETITION
• Duly accomplished application form
• Birth certificate issued by the NSO duly authenticated by DFA.
• Certificate of Naturalization.
• Identity Card (Cartão Identidade)
• Portuguese and old Philippine Passport (original and photocopy)
• Marriage certificate or Report of Marriage if outside the Philippines, if married
• Three (3) 2 X 2 photos (white background) with the following view: left side view, front view and right side view
• Processing fee of €45.00
