A legal document issued in Portugal in order to have any legal validity in the Philippines
must first be acknowledged or authenticated (“consularized”) by the Philippine Embassy.
Each document will bear the seal of the Embassy and the signature of the Authenticating
Officer. Please note that the Philippine Embassy does not assume responsibility
for the contents of the document.
Requirements:
• Duly Accomplished Affidavit Form
- General Affidavit
- Affidavit of Support and Consent to travel of a
minor child
- Affidavit of Support
- Affidavit of Abandonment
• Personal appearance of affiant/s
• Presentation of Philippine Passport, or any Philippine government-issued identification
card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID)
• Application fee: €22.50
Note: Affidavit of Consent to Travel of a Minor Child
Based on a DSWD Circular, a minor DOES NOT need a travel clearance if travelling
abroad (1) with either parent or with his or her solo parent or legal guardian;
or (2) whose parents are in the Foreign Service or living abroad or are immigrants,
provided he/she is holding a valid pass such as a dependent’s visa/pass/identification
card or permanent resident visa/pass/identification card which serves as proof that
he/she is living with parents abroad and their travel does not constitute child
trafficking.
