Procedure in the Police Station
• Request the assistance of a Police Officer to take the fingerprints of the applicant
• Use only black ink
• After the prints have been taken, the following information should be indicated in the NBI Form:
a) Name and Seal/Stamp of Police Station in the box labelled (Sundry Information)
b) Name and Designation of Police Officer who assisted in the box labelled (Sundry Information)
c) Signature of the Police Officer in the box labelled (Impressions taken by)
