TRAVEL DOCUMENT

A Filipino citizen who does not have a valid passport, for one reason or another, and wishes to return to the Philippines may apply for a travel document. This document is valid for ONLY one direct flight to the Philippines.

Requirements:

  - Duly accomplished Application Form
  - Affidavit of explanation (General Affidavit form)
  - Photocopy of old Philippine passport or NSO-certified Birth Certificate;
  - Three (3) identical passport photos size 4.5 cm x 3.5.cm, in color, with plain, white background:
  - Applicant must be wearing appropriate attire with collar and sleeves,
  - Travel Document Fee of €27.00
