FOR CHILDREN WHOSE PARENTS ARE BOTH SINGLE, BUT ARE ACKNOWLEDGED BY THE FATHER

Requirements:
• Duly accomplished ROB application signed by both parents – 4 sets
• Application fee of €22.50
• Birth Certificate bearing the child’s name, place and date of birth and the names of the parents (original and 4 photocopies)
• Passport of both parents (original and 4 photocopies)
• Duly accomplished Affidavit to Use Surname of the Father (original and 3 photocopies), Notarial fee of €22.50
• Personal appearance of both parents

FOR CHILDREN WHOSE MOTHERS ARE SINGLE AND ARE NOT ACKNOWLEDGED BY THE FATHER

Requirements:
• Duly accomplished ROB application signed by the mother – 4 sets
• Application fee of €22.50
• Birth Certificate bearing the child’s name, place and date of birth and the names of the parents (original and 4 photocopies)
• Passport of the mother (original and 4 photocopies)
