FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED BUT WHOSE MOTHER IS MARRIED TO ANOTHER MAN

Requirements
• Duly accomplished ROB application signed by the mother – 4 sets
• Application fee of €22.50
• Birth Certificate bearing the child’s name, place and date of birth and the names of the parents (original and 4 photocopies)
• Passport of both parents (original and 4 photocopies)
• Duly accomplished Affidavit to Use the Surname of the Father (original and 3 photocopies), if the child will adopt the surname of the father. Notarial fee of €22.50
• Duly accomplished Affidavit of Illegitimacy, original and 3 photocopies ; notarial fee of €22.50

REMINDER: Birth of Filipino citizens abroad must be immediately reported to the Philippine Embassy or Consulate General. Applications for Report of Birth after one year from the birth of the child shall require the execution and submission of an Affidavit of Delayed Registration of Birth, with a fee of €22.50.

AMENDMENT OF THE REPORT OF BIRTH (for purposes of acknowledgment by the father or legitimation of child)

A Report of Birth of a child can be amended for the following reasons: for children legitimated by subsequent marriage of parents; or for illegitimate children who were subsequently acknowledged by the father

Basic Requirements
• Personal appearance of affiant/s
• Four (4) copies of affiant/s’ Philippine Passport, or any Philippine government-issued identification card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID)
• Original and 3 copies of Report of Birth of Child
• Application fee: €22.50
