Additional Requirements:

For children legitimated by subsequent marriage of parents
• Original and three photocopies of Authenticated Marriage Certificate, in cases where parents’ marriage was solemnized in RP or Marriage Contract, in cases where parents’ marriage was solemnized at the Embassy or Report of Marriage in cases where parents’ marriage was solemnized outside of Embassy premises
• Duly accomplished Affidavit of Legitimation Form

For illegitimate children who were subsequently acknowledged by the father
• Affidavit to Use the Surname of the Father duly executed by the Father
